(a) Iliac bone osteomyelitis in an 18 year old female who presented with multiple discharging sinuses. (b) Despite being on uninterrupted supervised multidrug antitubercular chemotherapy for four months she continued to have pain, constitutional symptoms and discharge from sinuses. Her CD4 count at this stage was 453/cu. mm of blood. She was immunomodulated after four months of ATT. (c) Clinical picture 1 month after completion of immunomodulation. The sinuses healed and the count recovered to 1710/cu. mm of blood. ATT was continued for 18 months. (d) X rays at the end of treatment showing healing of lesions.
