Results Of Surgery In Habitual Dislocation Of Patella With Special Reference To Campbell's And Roux Goldthwait's Procedures (A Preliminary Study) S Sharma, D. K Jain, K. K Srivastava, Y Prabhakar, S. S Yadav April-June 1982, 16(1):30-35Thirty-four cases of habitual dislocation of patella are presented. This includes 30 patients who had undergone various operative procedures either on soft tissues, bone or both. A combination of two or more procedures in the same patient in comparison to a single procedure were found to be satisfactory and successful with a very low failure rate. In 11 patients combined Campbell’s and Roux Goldthwait, in 3 combined Campbell and Hauser’s, and in 2 elevation of hypoplastic lateral femoral condyle was performed in addition to combined Campbell’s and Roux Goldthwait or combined Campbell’s and Houser’s operation. Combined Campbell and Roux Goldthwait procedure was found to yield excellent and good results in 85.7 per cent cases. The overall results of operations in the present study with an average follow up of 5 years were excellent in 40 per cent, good in 46.6, fair in 6.6 and poor in 6.6 per cent of patients.

The Treatment Of Synovial Tuberculosis Of The Knee By A Method With Unrestricted Activities S. K Gupta April-June 1982, 16(1):14-18Of the 41 cases with Synovial tuberculosis of the knee 19 responded favourably to conservative treatment with anti-tuberculous drugs and knee mobilization exercises, 21 cases who responded poorly to a month’s conservative treatment were treated by synovectomy. Activity of the joint was not restricted during the whole course of treatment except in acute irritable phase or in the immediate post-operative period, and no appliances were used. It is concluded that in synovial tuberculosis of the knee antituberculous chemotherapy with non-restriction of joint activity resulted in restoration of normal joint function. Synovectomy should be restricted to the cases who do not show signs of improvement after one month’s chemotherapy.

Idiopathic Painful Heel V Marwah April-June 1982, 16(1):51-53Forty patients of idiopathic painful heel were observed closely. The cause of pain is considered to be neuralgia of medial calcaneal nerve as a result of entrapment or stretching while passing through the flexor retinaculum. Stretching is predisposed in patients with valgoid feet and odesity. Pain can be relieved by walking on outer border of heel of heel causing relaxation of the retinaculum or by superficial injection of one to 2 ml of xylocaine blocking the medial calcaneal nerves. ‘Calcaneal spur’ is thought to be only an association related to the process of aging. Relief obtained after operation for ‘calcaneal spur’ is considered to be due to sectioning of medial calcaneal nerves.

Open Wedge Osteotomy And Triceps Release In The Management Of Cubitus-Varus Deformity Hardas Singh Sandhu, Rakesh Madaan April-June 1982, 16(1):1-8One hundred and sixty cases of cubitus varus deformity, between 20 to 55 degrees, were operated. The cause was malunited supracondylar fracture in 156, malunited fracture of lateralcondyle in 2, and Erb’s palsy in 2. The olecranon and the bulk of triceps tendon moved medially on flexion of elbow when posterior surface of the lower part of humerus could be palpated under the skin. The deformity was corrected by open wedge osteotomy from the medial side in the supracondylar region. The osteotomy gap was plugged with full thickness wedge shaped bone graft. Pos-operatively 73.3 per cent obtained normal carrying angle, 18.7 per cent had cubitus rectus, and in 10 per cent 5 to 20 degrees of cubitus varus persisted. No serious complication was encountered.

Avascular Necrosis Of Femoral Head In Chronic Alcoholics S. S Babhulkar April-June 1982, 16(1):42-44A study of avascular necrosis of femoral head among 14 chronic alcoholics is presented. One patient had also an associated avascular necrosis of humeral head. The avascular necrosis of femoral head was seen in 47 other chronic alcoholics also but a known cause of avascular necrosis could be revealed in them. All the patients were males only. It was bilateral in 9 and unilateral in 5. The true aetiopathogenesis of the disease could not be established.

Functional Brace Treatment Of The Fractures Of The Shaft Of The Humerus R Bhalla, T. S Narang, L. H Lobo April-June 1982, 16(1):9-13Forty-four patients with 45 humeral shaft fractures were managed by functional brace method. All but one fracture united. The patients achieved excellent function of the limb by the time the fracture healed. Immobilization of the adjoining joints did not appear to be a pre-requisite for fracture healing, and functional brace provided sufficient stability to the fracture site for uninterrupted osteogenesis to occur.

Surgery In Non-Traumatic Dislocations Of Patella S. V Sharma April-June 1982, 16(1):24-29Surgical treatment for different types of dislocations of patella viz. (i) habitual, (ii) recurrent, (iii) congenital, and (iv) high riding patella is described. Satisfactory results were obtained by release of tight lateral structured alone (vastus lateralis, iliotibial band and lateral intermuscular septum, lateral patellar retinaculum and occasionally of rectus femoris) in habitual dislocations of patella. Construction of a check-rein or plication of the lax medial patellar retinaculum was considered unnecessary. Surgicap procedures for recurrent, congenital and highriding patellar dislocations are recommended.

Perthes Disease In Chotanagpur D. K Sinha, R. P Singh April-June 1982, 16(1):36-41A study of 51 patients of Legg-Calve-Perthes’ disease from 1976 to 1980 from Chotanagpur plateau is presented. A low incidence of the disease was observed as compared to western series. Peak age of the disease was found higher (7 to 10 years) in the present study. Bilateral involvement was however, similar to other western series. Average limb length discrepancy observed was 1.4 cm. A new classification of the has been proposed and plan of treatment accordingly is described.

Arteriographic Assessment Of Synovial Lesions S. S Yadav April-June 1982, 16(1):19-23Arteriography was done in 32 cases of synovial lesions including 5 with synovial sarcoma. It provided valuable information in delineating the mass and its source and pattern of blood supply. In synovial sarcoma neovasculature and pooling of contrast medium was observed.
