Patella has an important function both in flexion and extension, as it carries the extensor tendon away from the center of movements of the joint in all its positions. Therefore, every effort should be made to retain the patella. After excision of patella, extension is transmitted to the tibia through the lateral expansions. Greater the shortening of the central tendon of the quadriceps greater is the loss of quadriceps power because of the lax lateral expansion. Methods which aim at reassembly of the patellar fragments and maintenance of the reduction by means of stainless steel wire sutures passed through or around the fragments, screw either single or in pair, and tension band wiring. The results of tension band wiring in 15 cases of fractures of the patella are reported in this paper.
